Are you a compassionate and experienced Nurse Practitioner looking for a flexible role that allows you to make a direct impact in patients' lives? We're seeking a dedicated Family Nurse Practitioner (NP) to provide in-home assessments and care planning services throughout the Bronx, NY.
Assignment Details:
• Position: Family Nurse Practitioner (NP)
• Location: Bronx, NY (Hybrid Field-Based Role)
• Schedule: Flexible, Minimum 9 Hours Weekly
• Initial Patient Visits: Approximately 2.5 Hours Each
Requirements:
Active New York Nurse Practitioner License
Active NY Medicaid Provider enrollment
ANCC certification
NPI number and malpractice insurance
Minimum 2 years (or 3,600 hours) of NP experience in:
Family Health
Adult Health
Acute Care
Simple, Stress-Free Onboarding
