Registered Nurse - Home Health

$1996.48/week –

Job Summary:
Provides comprehensive home health nursing care to patients following physicians’ orders, ensuring quality and compliance with agency policies. Responsible for coordinating patient care, supervising home health aides and licensed practical/vocational nurses, and maintaining timely documentation.

Responsibilities:

• Assess patient needs and develop individualized care plans according to clinical protocols.

• Administer skilled nursing services and monitor patient progress during home visits.

• Supervise and evaluate home health aides, LPNs/VNs, ensuring adherence to care plans and regulations.

• Document patient interactions, treatments, and observations in HomeCare HomeBase within 24 hours.

• Maintain compliance with OSHA, HIPAA, and agency safety standards.

• Travel up to 75 miles to patients’ homes and participate in periodic office meetings and training sessions.

Qualifications:

• Hold a valid and current RN license in the state of practice.

• Have at least 1 year of recent adult home health or hospice experience.

• Proficient in HomeCare HomeBase (HCHB) EMR system; Oasis experience preferred.

• Possess CPR and BLS certification.

• Own reliable transportation, vehicle, drivers license, and auto insurance.

• Willing to travel within designated territory and accept fluctuating patient assignments.
