ADRC Intake Specialist

In this role, you aren’t just processing paperwork—you are a navigator for families in need. You will be the first point of contact, helping vulnerable individuals access life-changing services like the Community Care Service Program (CCSP) and Home and Community-Based Services (HCBS).

salary: $17.5 - $18 per hour

Responsibilities

The ADRC Counselor is the primary point of entry for seniors and individuals with disabilities seeking long-term support. You will conduct telephone screenings to determine eligibility for the Community Care Service Program (CCSP) and Home and Community-Based Services (HCBS).

• Assessment: Perform telephonic screenings to evaluate functional impairments and prioritize applicants for services.

• Waitlist Management: Assign applicants to appropriate program waiting lists and conduct mandatory re-screenings per Division of Aging Services (DAS) standards.

• Referrals: Connect clients to community resources, including medical professionals, personal care homes, DFCS, and Social Security offices.

• Compliance: Submit state-required forms and maintain accurate data in evidence-based databases. (RN Specialists review/approve CCSP denials).

• Advocacy: Report suspected abuse or neglect to Adult Protective Services (APS) or the Long-Term Care Ombudsman.

• Collaboration: Participate in quarterly provider meetings and assist with Area Plan development.
