Responsibilities:

• Provide personalized care and assistance to patients in their homes, including but not limited to bathing, dressing, grooming, and toileting
• Assist with daily living activities, such as meal preparation, light housekeeping, and medication reminders
• Monitor and report changes in patient condition to the Nurse, Physician, and family members
• Maintain accurate and detailed records of patient care and progress
• Collaborate with our in-office coordinator and field case manager to ensure seamless care coordination
• Participate in regular team meetings and training sessions to stay up-to-date on the latest best practices and industry developments
