Constant Care Health Services LLC is seeking a compassionate and dedicated Certified Home Health Aide to join our team in Brooklyn, NY, USA. As a key member of our home care team, you will be responsible for providing personalized care and assistance to our patients in the comfort of their own homes. Responsibilities:
- Provide personalized care and assistance to patients in their homes, including but not limited to bathing, dressing, grooming, and toileting
- Assist with daily living activities, such as meal preparation, light housekeeping, and medication reminders
- Monitor and report changes in patient condition to the Nurse, Physician, and family members
- Maintain accurate and detailed records of patient care and progress
- Collaborate with our in-office coordinator and field case manager to ensure seamless care coordination
- Participate in regular team meetings and training sessions to stay up-to-date on the latest best practices and industry developments
