POSITION SUMMARY:

Works under the direction of the Nurse Manager. Functions as a Team Leader. Responsible for planning, coordinating, and supervising all nursing activities related directly or indirectly to patient care for a group of patients within a specific clinical area. Evaluate patient’s physical and psychosocial needs, administers direct patient care, and provides education for the patient/significant other. Responsible for all nursing personnel assigned to the Team. Teaches and/or demonstrates nursing care procedures. Maintains a professional profile. Promotes cooperation, a positive attitude, and creates a safe environment for both patients and staff. Responsible for knowing and adhering to established hospital policies and procedures, including all aspects of safety.

JOB DUTIES & ESSENTIAL FUNCTIONS:

Assess the patient’s condition and nursing needs; coordinate nursing care activities with other health disciplines involved in the treatment effort to relate and integrate the nursing regimen with the treatment plan for patients. Develop a written individualized care plan which includes teaching needs and discharge planning. Administer direct care when professional nursing skills and judgment are needed. Administer medication. Practice infection control and participate in the maintenance of a clean and safe environment. Assumes responsibility and accountability for the delivery of nursing care to all assigned patients. Communicates changes in patient’s condition and questions regarding patient care to Charge RN/Clinician. Develops and maintains current written plan of care for assigned patients. Sets priorities to meet the patient’s needs. Sets patient goals and priorities with the patient and/or family. Provides document evidence in the medical record of patient/family teaching and their responses and understanding of the teaching. Ensures that patient education and instructions are consistent with that of the physician or designee. Communicates and works effectively with other staff and other departments. Participates in unit or hospital-based quality assurance activities or committees to contribute to the advancement of the nursing profession at SBSH.

REQUIRED EXPERIENCE AND QUALIFICATIONS:

Graduation from a School of Nursing. ( Under New York State Education Law § 6905 (commonly referred to as the BSN in 10), all Registered Nurses licensed on or after June 18, 2020, must obtain a Bachelor of Science in Nursing (BSN) degree within 10 years of their initial RN licensure to maintain their ability to practice in the State of New York. Failure to meet this legal requirement may impact your employment status .) Current Licensure to practice in New York State. Current BLS & PALS provider certification from the American Heart Association. Current ACLS provider certification from the American Heart Association required within 3 months of hire. New Grad need PALS and ACLS from the American Heart Association within 6 months of hire.
