The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health and Developmental Disabilities Division, Treatment Bureau.
The Behavioral Health and Developmental Disabilities Division (BHDD) aims to implement and improve a statewide system of prevention, treatment, care and rehabilitation for Montanans with mental disorders or addictions to drugs or alcohol. By teaming up with behavioral health specialists throughout Montana, BHDD provides substance abuse and adult mental health services for those in need of support. The Care Transitions Section of BHDD will provide Critical Time Intervention (CTI) services to Montanans experiencing a major life transition, specifically low-income Montanans transitioning from institutional settings. The Care Transitions section will coordinate care by establishing community partnerships and facilitating access to services and supports individuals need during their transition, helping them move into the least restrictive and most suitable environment where they can thrive.
This position is responsible for helping to research, develop, and subsequently implement a Critical Time Intervention (CTI) program. The position will report directly to the Care Transitions Program Supervisor.
The position will be responsible for the coordination of care for individuals experiencing major life transitions. The target population will include low-income individuals with serious or disabling mental illness (SDMI) as they transition from institutional levels of care to the community. This position will be responsible for managing multiple highly complex cases.

Qualifications for this Career Opportunity
- Knowledge of the characteristics/symptoms of SDMI in adults.
- Strong analytical and critical thinking skills.
- Skilled in managing timelines and managing the logistics of numerous complex cases.
- Ability to interact with diverse groups, providers, staff, and subject matter experts.
- Meet minimum qualifications:
- Bachelor's degree in a related human service field.
- Two years of experience in health, behavioral health, human services, or education field.
- Other combinations of education and experience may be considered on a case-by-case basis.

Additional Information
This position may be eligible for partial telework within the state of Montana. Specific conditions will be provided by the supervisor and must adhere to state policy.
This position may be eligible for a statutory $1.00 an hour base pay increase effective July 1, 2026.
This position is open until filled with frequent screening of applicants.
Most travel will be within normal working hours.
