Key Highlights Weekly pay: $2,185–$2,238 per week Guaranteed hours: 40 per week Contract duration: 13 weeks Shift options: 8:30–4:30 or four 10‑hour shifts (7:30 a.m.–5:00 p.m.) Start date: 07/06/2026 Travel assignment in a hospital setting with care management responsibilities Responsibilities Conduct assessments of patient needs to identify resources required for transition to the next setting and secure those resources Coordinate concurrent utilization management activities and participate in discharge planning across the patient care team Lead interdisciplinary rounds, care conferences, and planning to align care with patient goals and payer requirements Educate and collaborate with the care team and community resources to arrange durable medical equipment, transportation, and insurance authorizations Maintain clear documentation in the electronic health record and communicate effectively through standard hospital systems Requirements Active Registered Nurse license in Maine Associate or Bachelor degree in nursing 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review Utilization review and discharge planning experience required BLS certification required; CCM or ACM certification preferred Ability to work in a hospital inpatient setting and meet travel assignment requirements Benefits and Support Guaranteed hours per week Travel assignment with dedicated recruiter support from Healthforce Healthcare LLC Ongoing assignment support throughout the 13‑week engagement Pay Disclaimer Please note: Pay rates and guaranteed hours are estimates and may vary.
