This multidisciplinary team, located in our state-of-the-art building at 10 Members Way in Dover, NH, is comprised of board-certified endocrinologists, certified diabetes educators, registered dietitians, and specialized nursing staff who collaborate to deliver personalized care plans. Services encompass individualized and group education sessions focusing on blood glucose monitoring, nutritional counseling, exercise programs, and medication management. In addition to clinical services, the hospital provides community outreach through educational classes, support groups, and seasonal camps for children with diabetes. The program emphasizes a patient-centered approach, aiming to empower individuals with the knowledge and tools necessary for effective self-management and improved quality of life.We’re expanding our Diabetes Services team and are currently hiring Diabetes Educators for both 30-hour and 32-hour part-time opportunities!We're looking for a Registered Dietitian (RD) with a Certified Diabetes Care and Education Specialist (CDCES/CDS) credential who has experience providing diabetes self-management education, insulin pump therapy, and continuous glucose monitoring support. In this role, you'll partner closely with endocrinologists, primary care providers, and a multidisciplinary care team to help patients successfully manage diabetes through individualized education and ongoing support.Our 30-hour position follows a consistent schedule of Monday, Tuesday, and Thursday from 7:45 AM to 5:45 PM. We also have a 32-hour opportunity supporting our outpatient Diabetes Services program across multiple locations, with one day per week in Rochester and three days per week in Dover, working collaboratively with primary care and internal medicine teams.Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.Does this position require Patient Care?
