Job Responsibilities

Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Additional Duties: Provides care coordination/case management to children, birth to age 21, with specific health care needs Assists youth with transitioning from pediatric to adult health care Connects families with community resources Plans youth engagement/outreach events Assists in pediatric specialty clinics, as needed

Minimum Qualifications

Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification. Preferred Qualifications: Pediatric Nursing Experience Case Management Experience
