Transitional Care Clinical Pharmacy Manager (Hybrid Telehealth)
Montefiore Hudson Valley Collaborative
Montefiore Medical Center in Bronx, NY seeks a Clinical Pharmacy Manager for Transitional Care Excellence. This role leads pharmacotherapy for high‑risk patients, coordinating inpatient and post‑discharge care using telemedicine, RPM, and in‑person visits.
The position requires a PharmD, advanced residency, NYS licensure, and BCPS/BCACP preferred. Onsite with potential limited telemedicine after training.
