Job Description

We are seeking a compassionate and dedicated Registered Nurse (RN) with over 2 years of recent and relevant home care experience. The RN will be responsible for conducting 25-30 visits per week, including a mix of start of care and revisits. Strong proficiency in OASIS and Epic systems is required.

Responsibilities

Conduct patient assessments and implement care plans.
Perform OASIS documentation and ensure compliance with home health regulations.
Administer medications and treatments as prescribed.
Collaborate with patients, families, and interdisciplinary teams to optimize patient outcomes.

Required Experience / Certifications / Licensure

Active Registered Nurse (RN) license.
Minimum of 2 years of recent home care experience.
Experience with OASIS and Epic systems.
