Primary Care Nurse Practitioner or Physician Assistant to Practice in Avon, IN Join to apply for the Primary Care Nurse Practitioner or Physician Assistant to Practice in Avon, IN role at Indiana University Health Position Details Full-time, Day Shift Offering a 5- or 4-day work week No weekends, holidays, or call 100% outpatient Expected patient volume: 60-80 per week; diverse patient population of all ages Team: 6 physicians and 2 other advanced practice providers Onsite pharmacy support Located on the campus of IU Health West Hospital Key Responsibilities Assess health status, obtain relevant health history and comprehensive physical exam, preventative screening, and identification of medical risk factors. Formulate appropriate differential diagnoses and identify individual, family, and community needs. Develop treatment plans to maximize health potential of individual, family, and community. Order and adjust medication per collaborative written agreement and prescriptive authority in accordance with the Indiana Nurse Practice Act, or in consultation, and with signature of physician. Serve as a clinical resource for nursing staff, physicians, hospital, and other healthcare providers. Serve as an advocate for patients, staff, and management. Manage acute and long-term patient care.
