Crossroads Treatment Center
Crossroads Treatment Centers in Uniontown and Connellsville, PA is seeking a Care Manager to connect with patients monthly, assess needs, and provide resources to support recovery. You will build relationships with community partners, work with a dedicated team to deliver exceptional patient care, and participate in community events to promote Crossroads' evidence-based MAT approach.
