Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year in Adult Home Health or Hospice experience License: Must have a valid nursing license Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last 6 months Description: Provides coordinates and directs the provision of home nursing care according to physician orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses are expected. Must conduct home visits and participate in periodic office meetings. Reliable transportation, a current driver’s license, and agency-required liability insurance are necessary. Average number of patients per day: 5-8. Contractor will be assigned between 5-10 patients per day. Types of cases include start of care, wound care, and follow-up visits. Must be comfortable traveling up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation impacting billing and follow-up patient care may result in cancellation. There are no guaranteed hours with this contract. Clinician must hit weekly productivity points. All overtime must be approved in advance. On-call rotation required every 6-7 weeks. Local candidates accepted at the same rate. Must be open to traveling for patient assignments.
