Hospice Intake Coordinator

James River Hospice is seeking a Full-time Hospice Intake Coordinator to join our team! The Hospice Intake Coordinator manages the referral and admission process for patients entering hospice services. This role works closely with patients, families, referral sources, clinical staff, and leadership to ensure a smooth, timely transition into hospice care while delivering exceptional customer service and support. Schedule Full-time, in-office, 8:00 AM to 5:00 PM, with participation in a rotating weekend and holiday schedule.
