Summary The incumbent provides comprehensive psychosocial assessment‑triage and case management services to Veterans currently referred to specialty care through Care in the Community. The Social Worker will conduct clinical assessments, act as liaison, coordinate with Veterans and community providers, facilitate discharge planning, and assess Veterans’ and caregivers’ needs via telephone assessments. Qualifications United States citizenship (non‑citizens only if no qualified citizens). Master’s degree in social work from a CSWE accredited school of social work. Licensed or certified to practice independently at the master’s level in a state (GS‑11 grade requires licensure or certification; GS‑9 grade may be exempt pending completion of state prerequisites). Proficient in spoken and written English. Minimum one year of post‑MSW experience equivalent to the GS‑9 grade level in a healthcare or social work setting (clinical experience). A doctoral degree may substitute for this experience. Responsibilities Provide comprehensive psychosocial assessment, triage, and case management to Veterans referred to specialty care. Coordinate veteran care with specialty care teams, community providers, and discharge planning. Conduct telephone assessments of Veterans and caregivers to identify needs and strengths. Develop and monitor discharge plans and follow‑up needs. Act as liaison between Veterans, families, and VA/community resources. Provide clinical assessment and interventions, ensuring confidentiality and adherence to policies. Consult with interdisciplinary teams, staff, and vendors regarding psychosocial needs and treatment impact. Assist in orienting and mentoring less experienced Social Workers. Participate in on‑call rotations and adjust tour of duty to meet patient care needs. Maintain a valid, unrestricted driver’s license and operate a GSA vehicle for outreach. Duties and Benefits Work schedule: 8:00 AM – 4:30 PM. Competitive salary with regular increases; no sign‑on or relocation bonuses.
