VACCHCS Medicine Service / UCSF Fresno Department of Medicine – Physician, Palliative Care – Internal Medicine California – Department of Veterans Affairs Central California Health Care System, Fresno, CA, affiliated with University of California, San Francisco Fresno Residency Program. We are recruiting a full-time board certified or board eligible Internal Medicine physician who is palliative care trained. Qualifications Completed internal medicine residency. Medical degree (MD, DO, or equivalent) and eligibility to obtain a US medical license. Eligibility for appointment at the VA. Responsibilities Ensure high quality practices in end-of-life patient care through coordination and integration of care across inpatient, outpatient, sub-acute and long-term care settings, including the Palliative Care Consult Team. Serve as a consultant and resource for development and implementation of educational programs and measures to monitor program effectiveness. Assist patients and their significant others in active participation in care management and necessary lifestyle adjustments related to health and anticipated changes. Coordinate multidisciplinary care efforts for patients at the end of life across the continuum of care. Benefits Salary estimate: $240,000–$315,000.
