Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license. Must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred

Roles & Responsibilities
- Conduct clinical assessments to identify patient needs and determine appropriateness for care.
- Document and create comprehensive Plans of Care, progress notes, and clinical records.
- Develop and implement treatment programs for neuromuscular, neurological, and orthopedic disorders.
- Instruct patients and families on transfer techniques and the use of therapeutic equipment safely.
- Educate patients/families on therapeutic procedures that respect their age, culture, and learning readiness.
- Collaborate with interdisciplinary teams to ensure patient-centered care.
