Home Health Physical Therapist - Weekend SOC Visits

Please note that prior Medicare Certified Home Health and OASIS documentation experience is strongly preferred as most weekend visits are Start of Care Admissions.

What You’ll Do

As a BAYADA PT, you’ll help clients regain independence where they feel safest — at home. You will:

• Perform mobility, strength, balance, and safety assessments
• Develop individualized, evidence-based treatment plans
• Provide gait training, manual therapy, balance work, and therapeutic exercise
• Implement skin and wound-prevention strategies
• Educate clients and families to promote long-term success
• Document care using our intuitive EMR system

Compensation & Schedule

• Competitive weekly pay
• Flexible scheduling

• Mileage reimbursement or schedule-specific benefits
• Pay: $60-70 per point, based on qualifications

Qualifications

• Current Colorado PT license (or eligibility)
• Graduation from an accredited PT program
• Comfort with EMR systems (training provided)
