Case Manager

A community service provider in in Keene, NH is seeking a compassionate and organized Adult Case Manager to provide community-based case management and care coordination to adults experiencing serious mental illness, co-occurring disorders, and complex psychosocial needs. The Case Manager will help clients achieve greater stability, independence, and quality of life by connecting them with behavioral health, medical, housing, financial, vocational, educational, and community resources. This position works closely with an interdisciplinary treatment team and provides services in office, community, residential, hospital, and home-based settings as appropriate. Key Responsibilities Provide case management and care coordination services to adult clients. Connect clients with behavioral health, medical, housing, financial, transportation, vocational, and community resources. Coordinate care with hospitals, providers, residential programs, and community agencies. Assist clients with benefits, housing applications, transportation, and other resource needs. Support clients with recovery goals, daily living skills, medication adherence, coping strategies, and community functioning. Monitor client needs and respond appropriately to clinical concerns and crises. Participate in treatment team meetings, supervision, and case consultations. Maintain timely, accurate documentation in the electronic medical record. Follow HIPAA, Medicaid, OSHA, and agency requirements. Advocate for clients while promoting independence, self-determination, and recovery. Qualifications Bachelor’s degree in social work, psychology, counseling, rehabilitation, human services, or a related field; or Associate degree in a human services-related field with relevant behavioral health or community-based experience. Experience working with adults with serious mental illness or complex psychosocial needs preferred. Strong communication, organizational, problem-solving, and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Experience with electronic medical records preferred. Preferred Experience Community mental health or targeted case management Residential or psychosocial rehabilitation services Medicaid documentation and Targeted Case Management Care coordination with hospitals and community providers Trauma-informed and recovery-oriented care Additional Requirements Valid driver’s license and acceptable driving record. Proof of automobile insurance and reliable transportation. Ability to travel throughout the agency service area.
