The primary responsibility is providing a therapeutic environment for the Palmer Special Care Residence to maintain each resident's highest level of physical, social and physiological well-being. The Charge SCR LVN provides resident validation and effective redirection, anticipates the residents' needs and have effective communication with the cognitively impaired.Essential Job DutiesOrders and maintains medications.Keeps accurate maintenance of medical records.Schedules necessary service for residents on a daily basis.Coordinates with physicians and health agencies regarding the needs for Physical/Occupational Therapy, Lab works, and other special services.Assists with scheduling doctors' appointments and transport requests if necessary.During emergencies, takes proper action in accordance with approved emergency manual and notifies designated personnel.Responsible for completion and e-mailing of shift reports.Attends all mandatory facility in-service programs and any other mandatory meetings required by management.Attends Residential Health Care and SCR Department meetings as scheduled.Attends regular, on-going training sessions scheduled by the Director of Staff Development and Director of AL/SCR.Assists in the effective operation of Casa de las Campanas and participates in relevant facility activities.Responsible for assisting Director in the evaluating of SCR CNAs.Coordinates discharge of resident from Health Center back to SCR and schedules follow-up care.Ability to work as a member of a team and cooperate with co-workers.Contributes to a positive, healthy and safe working environment and accepts change in a positive manner.Performs any other relative, relevant duties as assigned per managers' request.When you enter a community managed by Life Care Services, you'll immediately feel the warmth of our Hospitality Promises:We greet you warmly, by name and with a smile.We treat everyone with courteous respect.We anticipate your needs and act accordingly.We listen and respond enthusiastically in a timely manner.We hold ourselves and one another accountable.We make you feel important.We embrace and value our differences.We ask, "Is there anything else I can do for you?"We maintain high levels of professionalism, both in conduct and appearance, at all times.We pay attention to details.All Employees must be able to pass background checks (fitness for duty physical, fingerprinting, employment references) as required by a licensed residential care facility.
