At RWJBarnabas Health , our Case Management teams work collaboratively to support patients and families navigating the social, financial, and psychological impacts of illness. As an essential member of our interdisciplinary teams, you will partner directly with physicians, nurses, and allied health professionals to ensure the emotional and social aspects of patient care are championed. As a Social Worker MSW, a typical day might include the following: Identifying, screening, and completing comprehensive clinical assessments for patients across assigned hospital populations. Formulating and implementing timely, appropriate, and safe discharge plans tailored to each patient’s unique needs. Providing supportive counseling to patients and their families regarding illness, coping with hospitalization, and care transitions. Connecting patients with critical community resources, medical equipment, or post-acute care, and conducting diligent follow-ups. Participating actively in interdisciplinary team rounds to review caseloads, align care paths, and advocate for patient needs. Coordinating social work services within the palliative care team and facilitating patient-family meetings for goal setting and advanced care planning. Serving as a liaison between hospital clinical teams, quality improvement, community hospices, home care agencies, and long-term care facilities. Entering accurate, clear, and timely clinical documentation and intervention records into the patient’s electronic medical chart. To be considered for this opportunity, you must have: A Master's degree in Social Work (MSW) from an accredited School of Social Work. An active Social Work Licensure in the State of New Jersey (LSW or LCSW). The flexibility to participate in a weekend and holiday work rotation alongside your team. Strongly prefer previous medical social work or hospital case management experience. Bilingual language skills in English and Spanish are highly preferred.
