A typical day may include visiting several patients across a defined geographic area, collaborating with a supervising Physical Therapist, and implementing individualized treatment plans aimed at restoring function, enhancing mobility, and promoting safety and independence. You will perform therapeutic exercises and functional mobility training, assist with gait and balance work, reinforce home exercise programs, and provide education on fall prevention and safe use of assistive devices.You will be part of a supportive interdisciplinary team focused on patient-centered outcomes, clear communication, and coordinated care. The home health setting allows you to see patients in their real-life environment, giving you the chance to make targeted recommendations about home safety, energy conservation, and equipment needs that directly impact their daily routines. You will work closely with nursing and other rehabilitation professionals to adjust care plans as patients progress, ensuring their goals align with both medical needs and personal preferences.Caseloads typically include adult and geriatric patients with a variety of diagnoses such as orthopedic conditions, post-operative recovery, neurologic impairments, cardiopulmonary issues, and chronic disease management. Visit lengths and daily patient volume are structured to allow for thorough, hands-on care and complete documentation. Because this is a community-based role, you will manage your schedule to accommodate drive times, patient availability, and visit frequency, with an emphasis on punctuality and reliability.Shifts generally follow daytime hours, with flexibility to accommodate patient needs and agency scheduling practices. Most work is performed Monday through Friday, with the potential for occasional weekend or holiday visits depending on patient volume and service needs. Within this framework, you will have the chance to develop strong rapport with patients and families, observing meaningful progress over time as they achieve greater independence at home.This position is well suited for a PTA who values autonomy in daily practice, appreciates a collaborative team structure, and wants to build experience in the expanding field of home health.
