The Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) connects older adults, individuals with physical disabilities, and their caregivers with vital programs, services, and support systems throughout Delaware. We're looking for a compassionate, detail-oriented Health/Human Services Case Manager I to join our Georgetown team and help Delawareans navigate options, access care, and find solutions that enhance independence, safety, and quality of life.

What You'll Do

• Assist clients with the DSHPP application process from start to finish.
• Make field visits to interview clients in their homes or communities.
• Make referrals and assist clients in applying for services.
• Create and manage activities in the WellSky Aging & Disability database.
• Communicate and coordinate with DSAAPD/Community Service Case Managers and Case Managers with the Division of Medicaid and Medical Assistance (DMMA).
• Utilize multiple systems for casework, including Outlook Calendar, TAP, AWW, Vital Statistics, and the Delaware Medicaid Enterprise System (DMES).
• Maintain accurate, timely case documentation in accordance with program requirements.

What We're Looking For

• Experience in case management, social work, or human services preferred.
• Comfort conducting in-person interviews and field visits with older adults and individuals with physical disabilities.
• Strong written and verbal communication skills for coordinating across agencies.
• Ability to learn and navigate multiple case management and state systems (WellSky, DMES, TAP, AWW).
• Strong organizational skills and attention to detail for accurate casework documentation.
• A valid driver's license and reliable transportation for field visits.
• A patient, compassionate approach to working with vulnerable populations.
• Ability to manage a caseload and work independently in the field.
