Join Priority Care Rehab Staffing 's SNF clinical team delivering skilled PT care to post-acute and long-term care residents. You'll work alongside an interdisciplinary staff focused on functional recovery and quality outcomes.

Responsibilities:

• Evaluate patients and develop individualized Plans of Care
• Provide therapeutic exercise, gait training, balance programs, and functional mobility treatment
• Maintain timely documentation in compliance with Medicare, Medicaid, and facility requirements
• Collaborate with IDT (physicians, nursing, OT, SLP) and participate in care conferences
• Supervise PTAs per NYS practice guidelines and support safe discharge planning

Requirements:

• Active New York Physical Therapist license
• Current CPR/BLS certification
• Prior SNF experience preferred
• Excellent communication, documentation, and team collaboration skills
