Hospice: Licensed Vocational Nurse (LVN) . Location: Washington, TX and surrounding area. Position: Full-Time or PRN. We're looking for a caring Licensed Vocational Nurse (LVN) to join our team serving the Washington and surrounding area. What You'll Do: Provide compassionate nursing care to hospice patients in their homes or care facilities. Complete routine patient visits and assist with admissions as assigned. Monitor patient conditions and communicate changes to the RN Case Manager and interdisciplinary team. Administer medications and treatments as ordered. Educate patients and families on disease progression, comfort measures, and medications. Maintain accurate, timely documentation using a company-provided tablet. Work closely with physicians, RNs, CNAs, Social Workers, Chaplains, and other team members to provide exceptional patient care. What We're Looking For: Active Texas LVN license. Hospice, home health, long-term care, or clinical nursing experience preferred. Strong communication and organizational skills. Compassionate, dependable, and patient-focused.
