RN Health Coach - Diabetes & Hypertension Care

UF Health

UF Health is seeking an RN Health Coach to work under physicians, collaborating with patients and Primary Care Providers to ensure quality, efficient, and cost-effective care. You will assess and plan care, integrate guidelines, and develop care systems to monitor progress and promote early intervention.

The role emphasizes autonomous practice, leadership, and effective communication within a dynamic health team. Requires RN licensure in Florida and 3–5 years of nursing experience.
