Hospice Social Worker

VIA Health Partners in Charlotte, NC is seeking a licensed clinical social worker to deliver comprehensive patient and family care within our hospice program. You will conduct psychosocial assessments, develop and implement care plans, coordinate with interdisciplinary teams, and provide education on end-of-life services.
