Field-Based Care Manager: Community Health & Discharge

Healthfirst in New York seeks a Field Based Care Manager to evaluate members' care needs and promote quality outcomes through field visits, hospital coordination, and telephonic support. You will develop care plans with clinicians, liaise with doctors, social workers, discharge planners, and community providers, and document all encounters in the member records.
