Primary Care Physician

Seeking a Primary Care Physician for a senior-focused PACE program serving individuals 55+ with chronic illnesses or disabilities.
Responsibilities include
- providing comprehensive primary care in coordination with an interdisciplinary team
- developing and integrating personalized care plans
- managing chronic and acute conditions
- conducting assessments and preventive services
- visiting patients in various settings and supporting staff
- participating in team meetings, quality improvement, and policy development
Qualifications include MD/DO, board eligible/certified in Internal Medicine, Family Practice, or Geriatrics, with at least one year of experience with elderly populations, and current licensing.
