Job Title/Position Hospice RN Registered Nurse Case Manager Reports To Director of Clinical Services Location Helena, Montana Sign on Bonus & Compensation $5,000 sign on bonus. Pay rate: $35 - $41 per hour. Additional on‑call pay. Job Description Summary Registered nurse plans, organizes and directs hospice care with emphasis on community health education/experience. The professional nurse uses community resources to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential Job Functions / Responsibilities Completes an initial, comprehensive, and ongoing assessment of patients and families to determine hospice needs. Provides a complete physical assessment and history of current and previous illnesses. Provides professional nursing care utilizing all elements of the nursing process as defined in the state Nurse Practice Act. Assesses and evaluates patient status by writing and initiating a plan of care, regularly re‑evaluating patient and family/caregiver needs, and participating in revising the plan as necessary. Initiates the plan of care and makes necessary revisions as patient status changes. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including patient and family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by the physician. Counsels patients and families in meeting nursing and related needs. Provides health‑care instructions as appropriate per assessment and plan. Assists patients with activities of daily living and facilitates efforts toward self‑sufficiency and optional comfort care. Acts as case manager when assigned by the Clinical Supervisor, coordinating patient care for the assigned caseload. Communication Completes, maintains, and submits accurate clinical notes regarding patient condition and care, including pain/symptom management changes. Communicates with physicians regarding patient needs, reports changes in condition, and obtains orders as required. Communicates with community health personnel to coordinate the care plan. Teaches patients and caregivers self‑care techniques, provides medication, diet, and other instructions, and offers health counseling. Maintains a safe environment for the patient. Assists patients and families in providing continuity of care. Works with family and the interdisciplinary group to meet emotional needs. Attends interdisciplinary group meetings. Additional Duties Participates in on‑call duties as defined by the on‑call policy. Ensures arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel, delegating responsibilities as required. Maintains personal growth, professional knowledge, and practice skills through continuing education and in‑service classes. Fulfills obligations for requested or accepted case assignments. Actively participates in quality assessment, performance improvement teams, and activities. Position Qualifications Graduate of an accredited nursing school; 1–2 years of recent acute‑care experience in an institutional setting and 1–2 years of recent home‑care experience. Current licensure in the state. Licensed driver with an insured automobile in compliance with state or organization requirements. Bachelor’s degree preferred; one year of hospice care experience preferred. Management experience not required; responsible for supervising hospice aides. Excellent observation, verbal and written communication, problem‑solving skills, basic math skills, and nursing competency. Physically able to walk, stand for prolonged periods, lift, position, transfer patients, and handle supplies and equipment. Adequate visual and hearing acuity required.
