Description
American Traveler is hiring an RN Case Manager for a Hospice position requiring a VA or compact RN license and recent case management experience.
Details
- Work as a Hospice Case Manager serving a geriatric, end-of-life patient population
- Average caseload of 5-7 patients per day
- Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
- No on-call responsibilities required
- Territory covers up to a 60-mile radius from the hub office
- Traveler expected to be flexible with assigned territory
- iPad provided for documentation; personal cell phone not used for work purposes
- Mileage reimbursement provided
Requirements
- Active VA or compact RN license currently in hand
- BLS certification required
- Minimum of 1 year of recent hospice case management experience required
- Netsmart EMR experience preferred
- Timely documentation and communication required
- THC drug screening required; medical marijuana cards may be accepted based on state laws
Additional Information
- Provide case management and end-of-life care coordination for geriatric hospice patients
- Scrubs required in any color; no jeans permitted
- Local candidates accepted at the travel rate
- No RTO permitted during the first two weeks and throughout orientation
- No call-ins permitted
- Orientation includes 16 hours of virtual training modules completed over two days
