Position Title: Family Nurse Practitioner/Physician Assistant

Department: Clinical Services
Reports To: Medical Director / Supervising Physician
FLSA Status: Exempt

Position Summary

The Family Nurse Practitioner provides comprehensive primary care services to adult patients, children and women's health services. Responsibilities include conducting physical examinations, developing assessment and treatment plans, ordering and interpreting diagnostic tests, prescribing medications, and counseling patients. Care is delivered in collaboration with a primary care physician and in alignment with NOELA Community Health Center's mission to provide high-quality, patient-centered healthcare in a culturally diverse setting. 

Responsibilities & Performance Standards 1. Patient Assessment & Care

• • Provide evidence-based primary care, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions for pediatric, adult, women and geriatric patients (scope may be tailored to clinic population).

  • Perform comprehensive health histories, physical exams, and appropriate diagnostic testing; interpret laboratory and imaging results and communicate findings to patients.

  • Develop and implement patient-centered care plans, including medication management, referrals, and patient education.

  • Provide preventive care services including well visits, immunizations, screenings, family planning, and health promotion counseling.

  • Collaborate with physicians, behavioral health providers, nurses, medical assistants, care coordinators, and other team members to deliver integrated, whole-person care.

  • Participate in quality improvement activities, including UDS-related measures, clinical performance metrics, and population health initiatives.

  • Utilize the electronic health record (EHR) for timely and accurate documentation, order entry, coding, and care coordination.

  • Participate in care team huddles, case conferences, and interdisciplinary meetings to optimize patient outcomes and clinic workflows.

  • Provide patient education that is linguistically and culturally appropriate; support health literacy and self-management.

  • Adhere to FQHC, HRSA, state, and federal regulations, including standards related to credentialing and privileging, collaborative/supervisory agreements, and scope of practice.

  • Participate in call coverage, extended hours, telehealth, and outreach services as assigned.

2. Quality Improvement Initiatives

• Participate in the implementation of clinic-wide continuous quality improvement programs.

• Identify and refer patients to appropriate community resources; provide guidance on utilization.

• Contribute to the development of improved community resources and care delivery patterns.

3. Additional Responsibilities

• Cross-train with support staff to assist as needed in patient care.

• Demonstrate a willingness to learn new tasks and seek clarification before beginning unfamiliar assignments.

• Participate in continuing medical education (CME) programs to maintain clinical knowledge and licensure requirements.

 

Minimum Qualifications

• Language: Proficiency in Spanish and Vietnamese is preferred for this position

• Education: Master's degree in nursing with a specialization in Adult Care (required) or Equivalent 

• Licensure/Certification:

  • Possess prescriptive authority.

  • Certified by the State Board of Medical Examiners.

  • Board certified by NCCPA, NBDPA, or other recognized certifying board.

• Experience: At least one year of full-time clinical nurse practitioner experience preferred.

• Skills:

  • Proficiency in physical assessment.

  • Ability to use standard computer applications, including word processing software.

  • Excellent oral and written communication skills.

  • Ability to work effectively in a culturally diverse, patient-centered, holistic environment.

• Other: Flexibility to work in multiple clinical settings strongly preferred.
