Hospice Medical Social Worker

Hospice Medical Social Worker Schedule: Monday - Friday, 8am to 5pm Location: Boulder & Boulder County. This role is primarily spent out and about in the community, meeting with patients and their families in home and facility settings. MSW/LSW Pay: $68,580 to $85,812 per year LCSW Pay: $73,354 to $91,750 per year Sign-On Bonus: $6,000 (fully paid within the 1st year after hire) Job Summary The Social Worker is responsible for providing psychosocial services for the team caseload. Provides direct services to patient and family, collaborates with the other Interdisciplinary Team (IDT) members within the care team and other health care providers in various care settings. Essential Duties Identifies the patient’s and family’s psychosocial, environmental, safety and developmental needs and participates in individualizing the patient’s plan of care based on those needs Incorporates patient/family preferences and goals within the plan of care Supports and respects the patient’s and family’s unique spiritual and cultural beliefs Facilitates family meetings Identifies community resources for the patient and families Participates in IDT, psycho-spiritual and other patient-related meetings; attends patient care conferences in LTC and AL facilities, as needed Education and Qualifications Graduate of an accredited Master’s Degree in Social Work program One year of experience in care setting that delivers end-of-life care, highly preferred Must possess CPR certification or be able to obtain within 60 days of hire Must successfully pass agency required background checks and drug screening Must possess a valid driver’s license, have reliable transportation, and auto insurance TRU's Benefits (Eligibility Varies by Position/Status) Paid Vacation time 401(k) with match FSA & HSA available Paid orientation Paid training Referral & signing bonuses (if applicable) TRU Community Care is an Equal Opportunity Employer #J-18808-Ljbffr
