RN Case Manager

RN Case ManagerProvides comprehensive, patient-centered nursing care in an outpatient clinic setting, supporting individuals and families across the continuum of care. This role utilizes the nursing process to assess, plan, implement, and evaluate care; administer medications and treatments; perform clinical procedures; and provide patient education and care coordination in collaboration with providers and interdisciplinary teams.
