The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individuals and families within their homes and communities.Essential Job Functions and ResponsibilitiesLicense to Practice as an LPN under the direction of the RN case managerCommunicates changes in condition to RN case manager and/or physicianParticipates in care coordination such as but not limited to patient/family meetings, goals of care meetings, interdisciplinary discipline team meetings, QAPI etc.Arrange/Order any equipment, supplies related to patient care needsRequest refill/reorder medications from pharmacy and/or physicianCommunicationPrepare clinical notes and updates the primary physician when necessaryCommunicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains physician's orders as requiredCommunicate with interdisciplinary teamPosition QualificationsGraduate of an accredited school of nursingAt least one year of hands-on clinical experiences as a nurse (in any field of nursing) Hospice experience preferred but not requiredCurrent license in state and CPR certification.Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
