Location Avera Therapy-Plaza 5-Sioux Falls Work Details Worker Type: Regular Work Shift: Day Shift (United States of America) Pay Range: $30.75 - $53.75 (actual pay rate dependent upon experience) Position Highlights This position is located in Sioux Falls, SD. Pay: Physical Therapist $39.05–$59.12; Occupational Therapist $33.83–$50.87. This position will accept either a SD licensed Physical Therapist or Occupational Therapist with lymphedema experience greatly preferred. $15,000 sign‑on bonus eligible with a 3‑year commitment. Student Loan Repayment: May be eligible for Avera’s Student Loan Repayment Program up to $10,000, $167 monthly from Avera. Brief Overview Responsible for the daily coordination and delivery of physical therapy services implementing specific treatment program for individual patients and patient groups according to the principles and practices of physical therapy and established objectives of the department. What you will do Evaluate assigned patients by gathering data from a variety of sources such as patient records, interviews, observation, and team members. Develop and implement individualized treatment programs for patients. Use selected activities and/or tasks to promote and maintain health, achieve treatment goals, and modify intervention strategies based on ongoing evaluation. Provide direct therapy services to patients using various functional settings and strategies, planning therapy within the patient’s capabilities and coordinating with the rehabilitation, medical, and nursing care plans. Prepare documentation such as evaluation results, individualized treatment plans, progress notes, and other reports. Communicate effectively with the patient, family, physician, and other disciplinary team members regarding the patient’s status, progress, and needs. Establish rapport and work cooperatively with interdisciplinary team members throughout the patient’s intervention program. Enhance professional growth and development through participation in educational programs, current literature, in‑service meetings, workshops, etc. Essential Qualifications The individual must be able to work the hours specified and perform each essential job function satisfactorily, including visual acuity, communication, hearing, and understanding speech and sounds. Required Education, License/Certification, or Work Experience Physical Therapist – Active license in the state of practice upon hire. Preferred Education, License/Certification, or Work Experience 1–3 years experience as a licensed physical therapist.
