HIV Social Worker - Community Outreach & Inpatient Support

Bayside Health - Alfred Care Group, as part of the statewide HIV Service, seeks a Social Worker to join the HIV Outward Program. You will provide psychosocial assessments, counselling, and care planning for people living with HIV in inpatient, outpatient and community settings. Working with a multidisciplinary team, you will support discharge planning, develop community linkages, and contribute to outreach with nursing, medical and mental health professionals. #J-18808-Ljbffr The Alfred Foundation
