Job Description
Title : Physical Therapist

Schedule : 8-hour shifts

Assignment Details :
- Contract Length: 14 weeks
- Guaranteed Hours: 32 per week

Requirements :
- Current State License as a Physical Therapist
- Proof of current CPR certification
- Current driver's license with reliable transportation and insurance
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience (preferred)
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities :
- Perform clinical assessments to identify patient needs and appropriateness for physical therapy.
- Document and review patient care plans, progress notes, and clinical records consistently.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on assessments and medical referrals.
- Instruct patients and families on safe transfer techniques and the use of mobility equipment.
- Provide patient and family education regarding therapeutic procedures based on individual needs.
- Collaborate with the interdisciplinary team to ensure comprehensive care for patients.

Compliance / Notes :
- Apply infection control and universal precautions during patient care activities.
- Accept clinical assignments aligned with education and competence.
