Job Description

Major duties include, but are not limited to:

• Provide evidence-based inpatient nursing care and care coordination across Tele-HM sites
• Manage patient transitions, including transfers and discharges, ensuring continuity of care
• Serve as point of contact for assigned sites and support orientation of new hub hospitalists
• Collaborate with hospitalists, interdisciplinary teams, Veterans, and caregivers
• Coordinate testing, labs, and care needs across VA and civilian facilities
• Identify and address actual or potential nursing issues; engage patients and families in care plans
• Participate in inpatient rounds and site-specific hospitalist meetings
• Collect and report program data, including volume, readmissions, and length of stay
• Communicate effectively with diverse populations and demonstrate strong customer service
• Prioritize tasks, adapt to changing demands, and maintain Veteran-centered care

Pay: Competitive salary, regular salary increases, potential performance awards
