Job Description
Inpatient Case Manager

Assignment Duration

This is a 13-week contract position.

Schedule

Shifts are Monday through Friday from 8:00 AM to 4:30 PM, with 7.5-hour shifts including a 1-hour lunch break.

Job Description

The Inpatient Case Manager will be responsible for comprehensive discharge planning and making patient referrals. The role involves working collaboratively with an interdisciplinary team and patients, primarily on a Med-Surg or Telemetry unit, while providing independent case management services.

Responsibilities

• Conduct discharge planning and make referrals for patients transitioning to home, community resources, or skilled nursing facilities.
• Collaborate with Social Workers and interdisciplinary teams to ensure effective patient care and discharge solutions.
• Round on the unit with physicians and the healthcare team to discuss patient needs and discharge plans.
• Manage a caseload of patients, typically ranging from 17 to 25 patients.
• Assist in obtaining resources such as housing and transportation for discharged patients.
• Utilize electronic medical records (Allscripts) for documentation and communication.

Required Experience / Certifications / Licensure

• Minimum of 1 year of inpatient case management experience.
• BLS certification required.
• PRI and SCREEN certifications (can be obtained online). Cost approx. $300 per certification.
• Knowledge of CarePort, MCG, and/or InterQual preferred.
• Experience in utilization review is a plus, but the primary focus is on discharge planning.
