We are seeking an experienced RN Case Manager Home Health to provide compassionate, high-quality nursing care to patients in their homes. This position offers the opportunity to build meaningful relationships with patients while working independently as part of a collaborative interdisciplinary care team.

Key Responsibilities

• Develop individualized patient care plans in collaboration with physicians, patients, families, and caregivers.

• Conduct comprehensive patient assessments and ongoing reassessments.

• Coordinate patient care with interdisciplinary team members.

• Monitor patients' clinical, functional, and emotional status.

• Communicate significant changes in patient condition to physicians and care team members.

• Review and update care plans and complete recertifications as required.

• Participate in case conferences and interdisciplinary communication.

• Provide patient and caregiver education on medications, diet, treatment plans, and self-care.

• Teach patients and caregivers appropriate home-care techniques.

• Document assessments, treatment goals, interventions, and patient progress accurately.

• Coordinate discharge planning and communicate discharge information to appropriate disciplines.

• Provide compassionate, patient-centered care while promoting independence and improved health outcomes.

Minimum Qualifications

• Active, unrestricted Registered Nurse (RN) license in Vermont and New Hampshire, or an unencumbered Compact RN license.

• Graduate of an accredited school of nursing.

• Minimum 2 years of recent Home Health experience.

• Strong clinical assessment, communication, and case-management skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.

• BLS/BCLS certification required.
