Responsibilities

• Administer prescribed medications and monitor patient responses according to care plans.
• Assess, document, and communicate patient status; report changes to the supervising RN.
• Provide basic nursing care and assist with activities of daily living (ADLs) for residents.
• Monitor vital signs, observe for changes, and respond to clinical alarms and call lights promptly.
• Collaborate with the healthcare team to implement care plans and ensure resident safety and comfort.
• Maintain accurate and timely documentation in charting systems; adhere to policies and regulations.

Requirements

• Active LPN license in Pennsylvania.
• CPR/BLS certification.
• Minimum 1 year of LPN experience in a long-term care or skilled nursing setting.
• Ability to work effectively in a fast-paced long-term care environment.
• Strong communication and documentation skills; ability to collaborate with the healthcare team.
