This position provides an exciting opportunity to practice patient-centered primary care within an integrated healthcare setting that combines medical and behavioral health services.As one of only a limited number of organizations nationally designated as both a Certified Community Behavioral Health Clinic (CCBHC) and FQHC, Liberty Resources offers physicians the opportunity to provide whole-person care in a highly collaborative environment focused on improving health outcomes for underserved communities.The successful candidate will work closely with primary care providers, behavioral health clinicians, nursing staff, care managers, and other healthcare professionals to deliver high-quality, evidence-based care to diverse patient populations.Compensation and BenefitsWe offer a highly competitive compensation package including:Base salary dependent on experience.Performance-based incentive opportunities.401(k) with employer match, 457(b) plan, and supplemental retirement options.Four weeks of vacation, eight personal days, and accrued sick leave.Paid CME time and reimbursement.Employer-paid malpractice coverage, licensure fees, and professional dues.Eligible site for NHSC and PSLF loan repayment programs.Manageable patient panels and carved out administrative timeResponsibilitiesClinical PracticeProvide direct patient care in an outpatient primary care setting.Deliver comprehensive medical services within your area of specialty, including Family Medicine, Internal Medicine, or Med/Peds.Diagnose, treat, and manage acute and chronic medical conditions.Perform preventive health screenings, annual wellness visits, and health maintenance services.Develop and implement individualized treatment plans based on evidence-based practices.Collaborate with behavioral health team members to provide integrated care for patients with co-occurring medical and behavioral health conditions.Participate in regular case conferences and interdisciplinary team meetings to coordinate care plans for complex patients.Coordinate care with specialists, hospitals, and community providers as appropriate.Maintain accurate and timely electronic medical records in accordance with organizational standards.Contribute to quality improvement initiatives and population health management efforts.Support care coordination activities that improve patient access, engagement, and health outcomes.Participate in backup telephonic on-call duty (Liberty Primary Care contracts with a service that takes first call).Demonstrate commitment to patient-centered, compassionate, and culturally responsive care.Stay current with clinical best practices and continuing medical education requirements.QualificationsMD or DO from an accredited medical school.Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Med/Peds.Active and unrestricted New York State medical license, or ability to obtain one prior to employment.DEA registration or ability to obtain prior to employment.Minimum of three years of clinical experience preferred.Experience working in primary care settings preferred.Strong interest in integrated care models and interdisciplinary collaboration.Commitment to serving underserved and vulnerable populations.Experience in an FQHC, community health center, or similar setting preferred.Excellent communication, teamwork, and patient engagement skills.Liberty Resources is committed to creating a diverse and inclusive environment and is proud to be an equal opportunity employer.
