Under the direction of the Director of Behavioral Health, the Manager of Integrated Behavioral Health and Case Management Services assists in the oversight of integrated behavioral health programming, develops and oversees integrated behavioral health education and training programs, and oversees the clinical and administrative supervision of the behavioral health integration specialists (BHS). This role also provides direct supervision to Fenway's Community Health Workers (CHWs)/Care Navigators/Care Coordinators, as assigned, and leads the development of an organization-wide case management program in partnership with nursing, primary care, Behavioral Health, and Accountable Care Organizations (ACOs). The Manager maintains a clinical practice, providing direct behavioral health counseling, advocacy, and referrals to community resource services, and carries a designated day of patient care hours each week.

Representative Duties:

Provides Direct Patient Care

• Maintains one day per week of dedicated patient care hours, structured according to departmental needs, which may include drop-in clinic hours, bridge appointments, or longer-term therapy visits.
• Depending on departmental needs, may carry a small panel of behavioral health clients.
• Follows the SBIRT model to provide psychosocial assessments and mental health and substance use evaluations for medical patients.
• Provides direct behavioral health treatment with an emphasis on short-term, evidence-based psychotherapeutic care within the primary care setting.

Oversees the Administrative and Clinical Supervision of Behavioral Health Integration Within Primary Care Teams

• Assists the Director of Behavioral Health with behavioral health integration program strategy, design, and development.
• Maintains awareness of industry best practices for deep integration of behavioral health expertise within primary care and medical team-based settings to support practice innovation initiatives.
• Ensures that newly hired behavioral health specialists receive a comprehensive orientation.
• In collaboration with nursing and primary care leadership, assists in the design of the daily operations of the full clinical care team, inclusive of care coordination, healthcare navigation, and health education informed by patients' social drivers of health.
• Makes employment-related decisions regarding assigned staff within the scope of defined authority and following established agency and program guidelines.
• Supports and continually improves the provision of professional, comprehensive, and effective clinical services for all BH clients.

Oversees Fenway Health’s Community Health Worker/Care Navigator/Care Coordinator Team

• Provides direct clinical and administrative supervision to Community Health Workers (CHWs)/Care Navigators/Care Coordinators, as assigned.
• Oversees daily operations, caseload management, and professional development of the CHW/Care Navigator/Care Coordinator team.
• Establishes and monitors performance expectations, workflows, and quality standards for CHW/Care Navigator/Care Coordinator services.
• Supports continuity and stability of the CHW/Care Navigator/Care Coordinator team, including onboarding, training, and coordination with clinical and medical partners.

Oversees Ryan White Reporting and HIV Case Management Coordination

• Partners with the Ryan White Project Director to ensure accurate and timely completion of Ryan White reporting requirements as assigned.
• Oversees CHW/Care Navigator/Care Coordinator responsibilities related to HIV-specific case management supported by Ryan White funding.
• Ensures CHW/Care Navigator/Care Coordinator documentation and service delivery align with Ryan White grant requirements and reporting timelines.

Leads Development of an Organization-Wide Case Management Program

• Leads the design and implementation of an organization-wide case management program and plan to address the social drivers of health impacting Fenway's patient population.
• Collaborates with nursing, primary care, Behavioral Health, and ACOs to align case management services across departments.
• Builds and leads a case management team structure that supports coordinated, cross-departmental patient care.
• Uses data to identify priority populations and to establish benchmark goals for case management effectiveness.

Oversees the Development of Population Health Based Behavioral Health Initiatives and Services

• Under the direction of the Director of Behavioral Health, and in collaboration with clinical departments leadership, has primary responsibility for developing behavioral health initiatives designed to improve health outcomes for identified patient populations (e.g., depression, diabetes, chronic pain).
• Makes use of data to establish priority populations.
• Collaborates with medical and quality improvement staff to establish benchmark quality goals for each priority population group and measures to evaluate behavioral health service effectiveness.
• Explores future opportunities for wellness-based integrated behavioral health programming.

Develops and Oversees Integrated Behavioral Health Educational and Training Program

• Creates and produces content for clinical training on behavioral health topics for medical teams in partnership with Clinical Leadership.

Participates in Activities Designed to Maintain and Enhance the Quality of the Behavioral Health Department's Programs and Services

• Participates in department management team meetings.
• Participates in health center quality teams when assigned.
• Acts as a liaison between the Behavioral Health department, clinical departments, and other agency departments as needed.
• Participates in other health center initiatives and committees as assigned.

Provides Training and Supervision to Assigned Staff

• Oversees hiring, training, supervision, performance management, and professional development of assigned staff
• Ensures that staff receive a comprehensive program orientation
• Ensures that staff receive sufficient training on the full range of their job responsibilities
• Conducts regular performance evaluations and provides ongoing feedback and coaching
• Makes employment-related decisions regarding assigned staff within the scope of defined authority as a manager following established agency and program guidelines

• Maintains professional competence necessary to perform job responsibilities; maintains and provides the agency with records of continuing education activities.
• Serves on agency committees and in professional organizations when requested.

Performs Other Duties as Related

• Independently licensed clinician (LICSW, LMHC or LMFT).
• Minimum of 7 years of relevant clinical experience, with at least 4 years post-licensure experience providing direct client care across clinical settings.
• Minimum of 2 years of clinical and administrative supervisory experience within a healthcare setting.
• Minimum of 2 years of experience with integrated behavioral health programming.
• Experience supervising community health workers, care navigators, care coordinators, or comparable non-clinical care team staff strongly preferred.
• Flexibility, adaptability, and ability to multitask in a team-based environment required.
• Demonstrated ability to work respectfully and effectively with individuals from a wide range of backgrounds, perspectives, and life experiences. Preferred Qualifications

• Experience in EPIC electronic health record preferred.
• Experience with Ryan White-funded programs or HIV case management preferred.
• Experience working with an ethnically, culturally, and racially diverse work staff.
• Ability to work harmoniously with diverse groups of individuals
