Wage: 18-23/hour

In this role, you will provide essential personal care and support to patients while being a valued member of the interdisciplinary care team.
What You'll Do
As a Home Health Aide, you will provide hands-on support to patients in their homes, helping ensure comfort, safety, and dignity. Responsibilities include:
- Assisting with personal care needs such as bathing, grooming, dressing, and toileting
- Supporting skin care and hygiene to prevent breakdown
- Preparing meals and assisting with feeding when needed
- Performing light housekeeping and maintaining a clean, safe environment
- Assisting with mobility, ambulation, and range-of-motion exercises
- Taking and recording vital signs as directed and within scope
- Assisting with self-administration of medications (as allowed)
- Observing and reporting changes in patient condition to the care team
- Documenting care and maintaining accurate records
- Participating in care team meetings and case conferences
- Supporting patient safety and proper use of equipment
- Participating in on-call, weekend, and holiday rotations
What We're Looking For
- Completion of a state-approved Home Health Aide training and competency program
- Listed in good standing on the state nurse registry
- Minimum age of 18 years
- Ability to follow written care plans and document services provided
- Ability to work independently with minimal supervision
- Compassion, patience, and strong interpersonal skills
- Commitment to maintaining patient dignity and confidentiality
- Valid driver's license with a reliable, insured vehicle
