Case Manager

Coleman Health Services is a nationally recognized nonprofit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community. We are seeking a MRSS Case Manager to join our team working throughout Allen, Auglaize, and Hardin Counties in Ohio (Based out of Lima). This position will require some on-call coverage throughout the week. MRSS Case Managers at Coleman work as a part of a cohesive, multi-disciplinary care team to provide the best outcomes for the clients and families we serve.The MRSS Team provides outreach to youth & families aged 20 and under to help them resolve a person-defined crisis and link with ongoing support services as needed. Employee Package includes: Hourly rate of $23.00 Health, dental, and vision insurance Flex Spending Account (FSA) Annual Wellness reimbursement Accrued paid time off (PTO) $2,000 staff referral bonus Supportive and collaborative team environment Strong commitment to providing high-quality care Job Duties: Provide short-term case management and wraparound services to youth and families aged 20 and under Transition clients and families into ongoing care Work collaboratively with treatment teams internally and externally to ensure safe transitions Minimum Qualifications: Bachelor's degree in a social service-related field OR 3 years' experience in a similar position Willing to get NPI and Medicaid Billing number (we'll help!)
