At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.Rn Case ManagerThe RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers.Responsibilities:Completes an initial assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illnessUses health assessment data to determine nursing diagnosisTriages calls and assists patient/primary caregiver with problem resolutionTeaches and counsels patients and families in meeting nursing and related needsMakes visits for symptom management and support as neededModifies patient treatment plans as indicated by patients' responses and conditionsMaintains current, accurate, detailed reports in accordance with policyMonitors, records and reports symptoms and changes in patients' condition to the IDGProvides health care instructions to the patient as appropriate per assessment and planObtains and receives physician orders as requiredPronouncement of patientsRequirements:Current license to practice as a registered professional nurse in GeorgiaCurrent and continued CPR certificationAt least one or two years of recent home care experience; hospice experience preferredGood verbal and written communication and problem solving skillsResponsible, valid driver's license; reliable, insured, automobile for making home visits required
