Employment Information

Conditions of Employment

I understand that by checking "Yes" to this question that if I am offered employment with Kingsport City Schools, I hereby consent to the collection and retention of my fingerprints as part of the application and employment process. I understand that my fingerprints will be used to check Federal and State criminal history records that pertain to me directly including, without limitation, the Federal Bureau of Investigation and Tennessee Bureau of Investigation, pursuant to 28 CFR, Sections 16.30-16.34. I acknowledge that I have been notified of the procedures to challenge the accuracy or completeness of my record, set forth in Title 28 CFR 16.34. I am aware that a copy of these procedures may be downloaded from FBI.gov.
3. I understand that knowingly falsifying information in this application shall be sufficient grounds for termination of employment. I also understand that if I am applying for a position as a teacher with Kingsport City Schools, knowingly falsifying the following information on this application shall be sufficient grounds for termination of employment and shall also constitute a Class A misdemeanor that must be reported to the district attorney general: (i) information regarding prior convictions, (ii) information regarding cause for prior dismissal from a teaching position, and (iii) information regarding submission of a written resignation to my current employer, if applicable.

6. If you are currently licensed to teach in the State of Tennessee, please state the grade bands and subject areas for which you hold licensure. (Endorsement numbers are acceptable.)
7. If you are not currently licensed in the state of TN, please state: a. Whether you hold an active out of state license and if so, which state and what you are currently licensed to teach in that state (subject and grades). b. If licensure is in process in the state of Tennessee and indicate the date you anticipate obtaining licensure and the license you are seeking (subject and grades).
8. Have you ever been discharged or non-renewed from any employment (including regular or extracurricular positions) in order to avoid discharge or non-renewal? If yes, explain circumstances, including the underlying facts, place, date and outcome.
9. Have you ever resigned or otherwise separated from any employment (inclusive of regular or extracurricular positions) in order to avoid discharge or non-renewal? If yes, explain circumstances, including the underlying facts, place, date and outcome.
10. Have you ever been placed on probation in any other employment capacity? If yes, explain circumstances, including the underlying facts, place, date and outcome.
11. Have you ever had any teaching or other professional license or certification, of any kind, revoked or suspended, or have you in any way been sanctioned by, or is any charge or complaint now pending against you, before any licensing, certification or other regulatory agency, or body, public or private? If yes, explain circumstances, including the underlying facts, place, date and outcome.
12. Have you ever been disciplined for misconduct by a past or present employer (by suspension, time off without pay, termination, etc.)? If yes, explain circumstances, including the underlying facts, place, date and outcome.
13. Have you ever been charged with, but not convicted of a crime? (Excluding civil infractions, such as minor traffic citations.) If yes, include explanation of the nature of the charge, place, date, and court. A pending criminal charge will not necessarily bar you from employment.
14. Have you ever been convicted of a misdemeanor or felony in this state or in any other state? (The term "convicted" means all adverse dispositions, including a finding of guilty, a plea of guilty or nolo contendere, an Alford plea, a stipulation to the facts, a deferred or suspended sentence, or a deferred prosecution. You may exclude disclosure of parking or moving traffic violations, if the maximum sanction provided by law for such violation does not include a period of confinement.) If yes, include explanation of the nature of the crime, place, date, and court. A conviction record will not necessarily bar you from employment.
15. Are you now being investigated for any alleged misconduct or other alleged grounds for discipline by any licensing, certification, or other regulatory body (teacher certification or otherwise) or by your current or any previous employer? If yes, you must provide the name, address and telephone number of the employer or licensing body and statement of the accusation against you.
16. Have you ever had a report of child abuse or sexual activities involving a PreK-12 student or minor filed against you with a school district, a federal, state, or police agency, or in court? If yes, explain circumstances, including the underlying facts, place, date and outcome.
17. If you are currently employed with another district, please indicate whether you have provided your current district with a written resignation and the date your resignation was provided. If you have not provided a written resignation to your current district, please indicate that you will provide a written resignation to your current district at least thirty (30) days prior to the beginning date of employment with Kingsport City Schools.
