We are seeking an Internal Medicine Specialist to serve as the primary medical case manager for complex and chronic cases within an established specialty and referral model. The specialist will collaborate closely with in-house general practitioners, surgical and cardiology specialists, trusted regional referral partners, technicians, and a dedicated referral coordinator to maintain clear referral pathways and coordinated care.
